Biosimilar insulins are an interesting, promising field, especially in India and other developing countries. The restrictions by the EMA and FDA are obviously high concerning immunogenicity etc. Biosimilar insulins should lower the high insulin prices. Transgenic plants are particularly a very smart technology.

The advantage of liraglutide is the once daily administration with a long biological halflife. The plasma halflife is longer and therefore the HbA1c-lowering effect pronounced, whereas the postprandial glucose levers are higher compared to exenatide. The LEAD-6-study showed a better HbA1c-lowering with liraglutide however, the comparison was made with twice daily 10 microgram Byetta versus 1.8 mg Victoza, which dose is also licensed.Hopefully the thyroid carcinomas seen in animals will not be found in broader populations in phase-4-studies. Cases of pancreatitis do not seem to occur more frequently with Victoza than in type-2-diabetics without this drug. Future observations will show if one of the two GLP-1-analogues might be preferred in special cases.

Besides Sitagliptin and Vildagliptin there will be now a 3rd gliptin available in Europe. Whereas vildagliptin officially needs monitoring of the liver enzymes, at least according to the P.I., and there are new warnings for pancreatitis for sitagliptin no such limitations are known to me at the moment. I do not think that there will be major advantages for saxa compared with sita or vilda, however, we will have to wait and see.

In a small subgroup of type-2-diabetes patients in the ACCORD trial which had been treated with Byetta (825 of 10 000) a lowerde death rate was found as compared to the whole group. This is a hypothesis-generating finding. Fewer hypoglycemias, even mild ones, may be an exüplanation for this result which, however, awaits bigger studies planned to answer this question
A new gliptin - what is the difference to sita- and vildagliptin?

As the member of an Advisory board for a gliptin ande not familiar with details on saxagliptin it would be interested to know what makes the difference to the gliptins already on the US and EU market. How is the DPP-4-specifity? Only for DDP-4 or also somewhat for DPP-9? Or other DPP´s ? What´s on with the transaminases? How often a day should it be given? Any other than sita- and vildagliptin regarding weight - only "neutral"?
