"Customized" intraocular lenses are gradually making their way into the mainstream of ocular surgery. First there were multifocal and accommodating IOLs. Now there are toric IOLs. And the future is upon us for more sophisticated designs such as the telescopic IOL.  As with any new technology, especially one that addresses such a specialized subset of patients, it will be some time before there is a consensus of opinion based upon a broad patient base.

The potential market for a newer and better optical solution to the problem of presbyopia is limitless.  Any optical system which functions with fewer "compromises" than those we have now will be a welcome addition even if its application is limited to just contact lenses alone. Broader applications to intraocular lenses and refractive surgery would be "icing" on the cake.

This article reveals what has been known on an "anecdotal" basis for a very long time in the arena of ophthalmic correction. While the increasing availability of designs in multifocal implants is a welcome addition to the surgeon's armamentarium, much as it is in the field of contact lens correction of presbyopia, monovision is often the more viable and desirable alternative. The most critical aspect in ensuring patient satisfaction, is careful history and screening.  The patients' visual goals and activities must be carefully probed to determine what distances are most critical in their daily lives.  This article is an important reminder that as new technologies are embraced, "old" ones still often have an important ongoing role.
