Pain is a complex of the personal, subjective, and unpleasant experiences  that may or may  not be related to physical injury or tissue damage.  It has been described “timeless, ineluctable, and disconcertingly indefinable.”  The expression may be influenced by psychosocial,  cultural,  and other factors. It is one of the “thorniest problems” this physician can face. Apart  from  induced abortion there is perhaps no medical topic that causes so much dispute as the nature of chronic pain.    The AMA Guides  to the Evaluation of Permanent Impairment note that  people tend to view pain complaints with suspicion and disbelief.  The American Pain Society expressed the opinion  that attempts to label chronic pain disorders as solely psychosocial issues appear to be misguided. The willingness to claim that chronic pain is psychological and therefore not as "worthy" as neuropathic pain has been a recurrent issue in the medical practice.  It is clear the much more research needs to be done.

Merck continues to have problems as a result of Vioxx.  It recently came out that the research studies which had been allegedly authored by independent medical scientists were actually ghostwritten by individuals paid by Merck. Many physicians have a poor opinion of industry sponsored research which is considered nothing more than a marketing tool. The editor of the New England Journal of Medicine was outraged when she learned the details of how the data was manipulated to provide a favorable outcome.

What is the best way to market pharmaceutical and other medical products to physicians? There is certainly bias among some physicians against the pharmaceutical industry. It is felt by many that industry sponsored research studies are invalid and will always show the products in a favorable light. On the other hand, many physicians welcome information from pharmaceutical companies. The problem here is often poor allocation of resources by the drug companies.
