As I have written elsewhere, P4P as a concept is really a given and, practiced well, is beneficial to patients and payors equally. The entire healthcare system benefits when evidence-based medicine is practiced and patients are treated according to the best science available, rather than by the "art" of medicine which is rife with variation.

President Bush requested as part of his 2008 budget that Medicaid adopt a form of P4P. The parameters and standards are vague and ill-defined, and will create lots of problems in implementation. The entire issue of P4P already has many variables; to tie reimbursement to a vague ill- thought -out version of P4P only spells disaster.

Canada is ahead of the US in budgeting and providing education for the use of HPV vaccination in teenage girls in a ne ffeort to reduce cervical cancer.
This is a significant public health issue that needs to be addressed from a public health point of view and not simply on religious/philosophical grounds as many in the US have proposed.
Merck and Company is first out of the gate with its Gardaril vaccine and is likley to continue to be a market leader even when competitiors like Glaxo Smith join the competition

In a surprise move TRIAD stayed public by being acquired by smaller rival Community Health System (CHS). This was a surprise to the industry because they thought TRIAD was positioned for a leveraged buyout and talks were ongoing with at least 1 private equity group.
This new association is unlikely to change services drastically for current TRIAD and CHS patients, but the new coalition will be watched for operating strategy as well as profitability
TRIAD's growth and aggressive acquisition could not be maintained indefinitely, so this is a good move for TRIAD shareholders

There is no doubt that P4P is here to stay. Given the escalating cost of healthcare, it makes sense to reward caregivers (physicians, hospitals and others) who provide the best outcomes and keep cost down.
The problem with P4P is that it generates strong opinions on all sides and stakeholders are eager to point out why "the other guy's version" WON'T work. It's time there was consensus on WHAT works and why P4P DOES work!

This is a welcome addition to the physician's arsenal- especially in emergency rooms and pediatric practices. However it must be interpreted with caution and does not replace clinical judgement. It is great to rule out the more deadly bacterial meningitis, but false positives could leave some cases of bacterial meningitis untreated if the physician relies solely on this test as a diagnostic tool.

Bad debt and weak patient volume continue to challenge hospitals, making them more innovative in operational efficiencies and managing growth. Going private is an option for some, and consolidation is bound to occur in the market place as well.

GE's acquisition of Abbott's invitro diagnostic and point of care divisions arose from a new acquisitive fervor of General Electric (GE), as well as the need for Abbott (ABT) to be more profitable. This is a good move for both companies, but we must wonder what other changes there will be coming down the pike and how traditional customers of both companies will be affected.

US healthcare is an expensive hodge-podge of services which was born out of a post-World War paradigm that no longer applies. Each "patch" applied just delays the inevitable. Healthcare will not be affordable and universally available until the public demands it. Corporations are interested in a "fix" not so much because of altruism, as much as the cost of employer-driven healthcare.
